Registration Form for Open House

The Department of Chemistry invites prospective graduate students to attend one of our two Open Houses this year (March 28-29 and April 18-19). Please fill out this form to register for the Open House. We will also need you to send an unofficial transcript from your most recent University as well as at least one letter of recommendation to:

You may also send them electronically to muisener@cas.usf.edu. We need to receive these materials at least two weeks in advance of the Open House in order to make your reservations.

Once you have registered for the Open House, and we have received the unofficial transcript and one letter of recommendation, we will be in contact with you to make the arrangements (transportation and hotel).
