Travel Forms

The USF Travel Program provides all travel related forms for download from their website. Starting July 1, 2003 when traveling, please use these forms. NOTE: The forms available for download at The USF Travel Program's website are currently not up-to-date. Please use the forms listed below.

Forms for Volunteers in the Lab

The following three forms need to be filled out when you have volunteers working in your lab.  Please keep a copy of the three forms and submit a copy to the Department attention Patricia Muisener.  The Department will then submit a copy to EH&S and HR.  All three forms must be completed!

Appointment Processing

The Appointment form is needed for all Research Assistants, Post-docorates or hourly OPS, for all new appointments or for any change to an existing appointment.  The form should be completed, printed out, SIGNED, and submitted to the department accountant (John Connaughton).

This is an Excel form and it has no protections attached to it.  You can fill it in electronically or print it first and do it manually.  A couple of things you may not know (eg., USF employee ID or student U number), and if you do not, don't worry about it, because we do.  But if you do not provide an adequate who, what, where, when and why then we will not be able to complete the appointment until you do.  Thanks!

Division of Sponsored Research

Accident, Fire, or Hazard Report Form

Use this form to report the occurrence of an accident, fire, or hazard.  The form must be completed, printed, and submitted to the Safety Committee, C/O Chairman, SCA 400 for processing.

Report of Outside Activity

From the link below: Outside Activity means "any private practice, private consulting, additional teaching or research or other activity, compensated or uncompensated, which is not part of the employee's assigned duties for which the University has provided no compensation."

P-Card Form

Please use the below form for all P-Card purchases. Instructions for P-Card usage are also listed.

Materials and Supplies Ordering

Use this form to request the odering of materials and supplies. The form must be completed, printed, and submitted to the Storekeeper/Receiving Clerk for processing.

Obtaining Solvents, Gases and Dry Ice

These forms need to be filled out with signature from the PI in order to obtain solvents, gases or dry ice.

Requisition Request

If you want someone to do a requisition for you, this form must be filled out or the requisition will not be done.

Faculty-Authored Textbooks

A faculty member who is the author of a textbook, book, software, or collateral materials and who requires the use of that material in his or her course or courses, must inform the Provost if more than $500 is received in one year from the required use of the textbook in his or her class. The faculty member must certify that the required text is the only text that is uniquely suited for use in the author's class. The number of students expected to enroll in the class for the year should be included. The above reporting requirements also apply when the faculty member assigning the materials is a relative of the author, a member of a teaching team of which the author is a member, or if the author is in a position to require the materials in any University course or program. Sec. 112.313, F.S.
The form below should be prepared and approved by the appropriate committee and the Chairperson.
